I actually did a study on this at work in my lab. Rehydrating in warm tap water by sprinkling on top of the water, waiting 15 minutes, then stirring produced the highest viability 98%+ every time. You can get the viabilty down to 75% if you use cold, DI water and shake vigarously immediately after dumping yeast in. You don't want to use wort to rehydrate in because it will cause osmotic shock.
