AHA Competition Form

Is there an AHA competition form available that is in a word or some other editable document? All I've been able to find are the .pdf files. My handwriting sucks and I wanted to type in the info.

I have a generic PDF entry form with bottle labels that I use when a comp doesn't provide them online. There are form fields in it so you can type your information rather than writing, even if all you have is Acrobat Reader. As you fill in the top section, the bottle labels get filled in automatically.

I don't remember why there are 3 versions, they all look pretty much the same to me.
