The post O-rings develop cracks over time and can cause such leaks. I replace mine about once a year whether they need it or not. As mentioned earlier a very light touch of keg lube on the O-ring once in a while helps preserve it.
Also, have you checked the disconnect itself? You should take them apart every now and then and clean them. An amazing amount of crud can build up inside them. They are easy to take apart. Just stick a screwdriver in the slot on top and screw out the top piece. Everything inside with then just drop out. Do pay attention to the order they come out and which end you need to stick back in first.
