I boil 3 quarts of water and put it in the mash tun. I leave it until I'm ready to add strike water then dump it. I heat the strike water 10 degrees over and I nail it every time. Of course, my grain bill has been around 11lbs pretty much every time.

My first guess is that your thermometer has something wrong with it.

Yikes! I hope not. I got tired of inaccurate, unrelible thermometers and shelled out some real money for a lab grade instrument. It cost more than my grain mill. I've always trusted it, so I never double check. I will calibrate it against another before I brew again.
